SEBI ISR Forms: Which One Do You Actually Need?
SEBI prescribes a set of forms for holders of physical shares. They overlap enough to be confusing, and sending the wrong one costs weeks. This page maps each situation to the form that actually applies.

In short
ISR-1 updates KYC. ISR-2 gets your signature attested by a bank. ISR-3 opts out of nomination. ISR-4 covers duplicate certificates and other service requests. ISR-5 is for claims after a shareholder dies. SH-13 nominates someone and SH-14 changes or cancels that nomination.

What to Attach
- Self-attested PAN of every holder — needed for essentially every form
- Aadhaar or another accepted address proof
- Cancelled cheque with your name printed, or a bank-attested statement
- Demat account statement showing DP ID and Client ID
- The share certificate, or any dividend counterfoil if the certificate is lost
Where to Send It
All of these forms go to the Registrar and Transfer Agent handling that company's shares — never to SEBI, and generally not to the company's head office. If you do not know the RTA, the company's investor relations page names it.

Frequently Asked Questions
Which ISR form do I need if I have just moved house?
Form ISR-1, with address proof matching the new address. Do it promptly — a stale address is the most common reason dividends go undelivered, and seven years of undelivered dividends sends the shares to IEPF.
What is the difference between the suspense account and IEPF?
A company's unclaimed suspense account holds shares the company could not deliver, and you claim them from the company via ISR-4. IEPF is a government fund that receives shares after seven years of unclaimed dividends, and recovery there requires Form IEPF-5. Different custodian, different form, different timeline.
Do these forms apply to demat holdings?
Largely no. The ISR set exists for physical securities. If your shares are already dematerialised, most of these requests go through your depository participant instead.

From the website of the RTA that holds your folio, or the investor section of the company's own website. Always use the current version from the RTA handling your case, as some attach their own covering instructions.
